The purpose of this paper is to provide a brief overview of housing resources for older adults that may be available to assist MFP programs in providing housing options for these participants.

The purpose of this Toolkit is to provide examples and tools to assist individuals transitioning under the Money Follows the Person (MFP) program in sustaining housing and succeeding as a tenant.

This report synthesizes the Washington Money Follows the Person (MFP) experience and presents it as a model with actionable recommendations for other state MFP programs on finding housing champions, understanding housing needs in the transition process, creating a housing-focused job description, training housing specialists, supporting team work, and building communication and collaborations.

New Editions provides logistics and management support to assist the OD in implementing the Call to Action Strategic Plan. We plan and conduct important events: a conference with major health care provider organizations to identify best practices in disability awareness training for physicians and nurses and a forum with employers to identify best practices in providing access to health care services for employees with disabilities. New Editions also develops publications for employers and employees to support improved health care access in the workplace.
