Formative Research Report on Youth with Special Health Care Needs in Transition

This report presents the findings of an innovative, qualitative study exploring the experiences of youth with special health care needs as they transition into adulthood. It focuses on the importance of including youth in the decision-making process, and uses their insights and experiences to help shape and inform recommendations for future initiatives.
